We accept all major credit cards, personal checks with ID, and cash.

Should you need financial assistance here are a few options:

We can split the Total Amount Due into 2 payments.  Half is due on the day of service and the other half is preauthorized and held for 30 days.

Write a letter stating that they give you permission to use their (check or credit card) in our office, date the letter and sign.  We also need a copy of their driver’s license and copy of credit card.  Please write the credit card number on the letter and expiration date.  Fax the letter to 770-516-4826
